Important note:​

Dear Valued Customer,

 

In an effort to improve our repair performance and facilitate faster remediation of products to patients, we are updating the return instructions - requiring you to segregate your products by group (as described below) and request a single collection for each group.

 

1.    Gather your products and segregate them by group:

      a. Group DreamStation (DreamStation CPAP & ventilation units: CPAP Auto, BiPAP Auto, BiPAP ASV, BiPAP ST, BiPAP AVAPS, 

                excluding DreamStation GO)

      b. Group Trilogy (Trilogy 100/200)

      c. Group Others (A-series, 60 Series, Dreamstation Go, E30, Torch, Dorma, Omnilab, 50 Series, Legacy Bipap)

2.    Remove all accessories (including  humidifier, modem, SD card, electrical cable, carry bag, etc) and keep them

3.    Package securely by group of products (with NO accessories) using the box from the replacement product (see photos below)

4.    Fill the form below for one collection of a product group and submit to generate a customer reference number

5.    Add the customer reference number you get at the end to your pallets or parcels

How many total boxes/cases

We are looking for how the units are packaged, how many boxes/cases are these individual units packed within. Return Labels have a maximum weight of 20KG, so you will need to package them ensuring the boxes/cases do not exceed this.

How many Pallets (if applicable)

For customers with large quantities requiring vehicle pickups, please advise how many pallets you have for collection (assuming EU pallet size). If you require a pallet collection please make sure the pallet does not exceed 1.8 metres in height and is appropriately protected (wrapped at least 5 times) for secure transportation of the products.

Preferred Collection Date

Required

Please note a preferred collection date is only required for pallet collections. If you are registering a parcel collection this is not needed and we aim to collect your parcels within 4 working days of registration.

Special Loading Requirements

If you have special loading requirements, such as Forklift, parking arrangements, etc - please advise 

Serial IDs

Please copy and paste the full SERIAL ID list here for the devices included in this shipment, as this will be used to create an inventory report for the shipment. One Serial ID per line. You may add multiple batches of Serial IDs by hitting the pre-process button after each batch. All Serial IDs provided will be grouped in the table below. Once you have provided all your Serial IDs please submit your request at the bottom of the page.

  •  All accessories should be retained by the customer as these maybe required for use with a  future repaired replacement device. Any accessories sent back to Philips WILL NOT be returned

  • Products returned to Philips must be securely packaged to avoid damage during  transportation
